Care Coordination 4 Children 

Description:

Provides home visiting services for care management, health assessment monitoring, referral, and parenting information. Click here for Referral Form.

Eligibility:

High risk infants and preschoolers (ages 0 - 5 years old) who meet the CC4C service conditions.  

Fees:

No fees assessed to families.

Service:

School Health Nursing     

Description:

Provides nursing services to the students and staff at all schools in the Durham Public Schools System. This includes students in the Exceptional Children Program and their staff, as well as students in Alternative Educational Programs.  Services include developing individual care plans for students with chronic health concerns such as asthma, diabetes, seizures, sickle cell, etc. School Nurses serve as the health resource persons for teachers and other school staff.  They promote appropriate decision making in order to impact on the teenage pregnancy and infant mortality rates.  Classes are also taught on health related topics.

Students, staff and faculty of Durham Public Schools

Each nurse is assigned to several schools; messages for nurses can be left at the Durham County Health Department 8:30 a.m. - 5:00 p.m. Monday-Friday or at the local school.
